29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about October 11, 2019, the employer failed to report to OSHA a work-related in-patient hospitalization. The in-patient hospitalization occurred on October 11, 2019 and was not reported to OSHA until October 30, 2019.
