29 CFR 1910.119(e)(3)(v): The process hazard analysis did not address the hazards related to facility siting: Avantor Performance Materials/Phillipsburg, NJ/Building 101-Methanol Distillation Process Area: a) On or about October 2, 2019, the 2016 PHA Revalidation of the Methanol Distillation Process did not address the facility siting hazards of potential methanol vapor cloud explosion (VCE) scenarios and the associated over pressure impact on occupied permanent buildings, exposing the employees to potential fire and/or explosion hazards in the event of a Methanol release.
