29 CFR 1910.147(a)(2)(ii)(B): Alternative measure(s) to provide effective protection were not developed and implemented where employees were required to place any part of their body into an area on a machine or piece of equipment where work was actually performed upon the material being processed (point of operation) or where an associated danger zone exists during a machine operating cycle. ESTABLISHMENT: The employer did not ensure that alternative energy control procedures were developed and implemented where employees were required to perform routine maintenance and servicing of machines, such as (but not limited to) bell paint machine. On or about September 23, 2019, while an employee was installing a bell support peg, the employee's left pinky finger got caught in the chain and gear assembly, resulting in a partial amputation of the finger.
