Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by injuries from inadequately labeled and maintained racking systems: (a) Racking throughout the facility - On or about October 29, 2019 the employer did not ensure regular inspections for damage of the racking systems exposing employees to struck by hazards. (b) Racking throughout the facility - On or about October 29, 2019 the employer did not display in one or more conspicuous locations a plaque to permanently and prominently display the permissible loading of the rack exposing employees to struck by hazards. (c) Racking throughout the facility - On or about October 29, 2019 the employer did not ensure that the rack columns were anchored to the floor with anchor bolts adequate to resist all applicable forces exposing employees to struck by hazards. (d) Racking at rear of building by exit door - On or about October 29, 2019 the employer did not ensure the affected areas of damaged racking was immediately isolated exposing employees to struck by hazards.. (e) Racking at rear of building by exit door - On or about October 29, 2019 the employer did not have a storage rack design professional evaluate the damage, and unload, replace or repair if directed by the professional, any damage columns, beams, or other structural components to restore the system to at least its original design capacity exposing employees to struck by hazards. (f) Racking throughout the facility - On or about October 29, 2019 the employer did not require all pallets to be maintained in good, safe, operating condition, ensure that pallets were properly placed onto pallet load support members in a properly stacked and stable position, and prohibit double-stacking of any pallet position, including the top most position exposing employees to struck by hazards. (g) Racking throughout the facility - On or about October 29, 2019 the employer did not ensure the bottom of all columns were furnished with column base plates exposing employees to struck by hazards.
