29 CFR 1910.23(c)(11): Portable ladders used to gain access to an upper landing surface did not have side rails that extend at least 3 feet (0.9 m) above the upper landing surface. This violation occurred on or about October 21, 2019 and at times prior thereto, the employer does not ensure that the side rails of the ladders, used to access flatbed trailers, are more than 3 feet above the landing of the trailers. This exposes the employees to the hazards of falls. Valmont Industries, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.23(c)(11), which was contained in OSHA inspection number 1228075, citation number 1, item number 1 and was affirmed as a final order on November 15, 2018, with respect to a workplace located at 801 N. Xanthus Ave, Tulsa, OK 74110.
