29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: Paper Machine #9, Stock Prep- Employees were not protected from a struck-by crushing hazard where bales of paper were not stacked in a manner that would prevent the bales of paper from falling. On or about October 21, 2019 an employee was fatally struck by a paper bale weighing approximately 1200 pounds, with dimensions of approximately 60 inches wide, 32 inches deep and 40 inches tall. It had been stacked on top of two similarly sized paper bales so the stack was approximately 120 inches or 10 feet tall. The bale beneath the one that fell off the stack and onto the employee was sloped in the direction that the bale fell when it struck the employee who was working next to the stack of bales.
