29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing, or blocking of machines or equipment from energy sources: a. On or about 1/29/2020, at Interplex Sun Belt Inc., 6690 Hiatus Rd, Tamarac FL 33321. Molding department - An employee was not provided with hardware for controlling hydraulic energy while setting up and/or servicing a molding press (i.e. M-18, Nissei Model TH40-54SE), which exposed employee to a caught by hazard. b. On or about 1/29/2020, at Interplex Sun Belt Inc., 6690 Hiatus Rd, Tamarac FL 33321. Production Area - An employee was not provided with hardware for controlling energy while servicing pneumatic cylinders, which exposed employee to a caught by hazard.
