29 CFR 1910.29(b)(2)(i): (i) Midrails are installed at a height midway between the top edge of the guardrail system and the walking-working surface; Employees were exposed to falls from heights of 4 feet or more as follows: On or about October 18, 2019, the employer failed to ensure midrails were installed on the stairway used for employee access to the outside aggregate pit. Employees were exposed to falls of approximately 10' 3" to the ground below. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)
