29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) On or about 10/21/19 on the 36" Roan flexible packaging machine; an employee, making an adjusting to the web during processing, was exposed to an in-going nip point when the gap on the inlet side of the guard on the draw rollers was measured at 3/4". b) On or about 10/21/19 on the 48" flexible packaging machine; employees were exposed to an in-going nip point hazard when the gap on the inlet side of the guard on the draw rollers was measured at 1/2". ABATEMENT DOCUMENTATION REQUIRED
