Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees working from the ground were exposed to a struck-by and crushing hazard associated with loading pipe onto a trailer: Route 101- On or about October 4, 2019 an employee was struck by and pinned beneath a pipe that rolled off of a trailer being loaded by Komatsu WA320-7 wheel loader with a JRB construction utility fork attachment. The trailer being loaded with one 50 foot long, 16 inch diameter, 34 pounds per foot HDPE pipe was not level, in that the pipe was loaded on the high side of a sloped roadway with the injued employee positioned on the low side of the trailer where the pipe rolled off of the trailer.
