29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees. At a trench located at 5900 Cromwell Marine Creek Rd., Fort Worth, Texas; an employee working in the trench was not provided a safe means of egress while looking for a fiber optic line in a approximately 5-foot 5-inch deep, 54.5-inch wide and 12-feet long trench was exposed to cave-in and crush-by hazards.
