29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. a) In the production area, on or about October 2, 2019 and at times prior to, the employer did not develop and ensure the use of equipment specific lockout tagout procedures for employees who performed maintenance work on the JB17 extruder, thus exposing employees to amputation hazards.
