29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about October 2, 2019, at a job-site located at 3322 Clarkmill Rd, Norton, Ohio 44203; the employer did not ensure that an employee worked at all times inside the existing protective system in place in an approximately 14 feet deep trench. An employee was injured by a shifting road plate when he stepped outside the existing protective system inside a trench with near vertical side walls, type "C" soil and a depth of approximately 14 feet."
