29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: The controlling contractor is failing to ensure that all employees of the sub-contractor Clean & Secure Roofing LLC are protected from fall hazards associated with using ladders during residential construction. This was most recently documented on or about October 7, 2019 at 9223 Dorcas Street, Omaha, NE 68124. Four employees from Clean & Secure Roofing LLC utilized a ladder that was not extended 3 feet over the upper landing surface.
