29 CFR 1910.146(d)(3)(vi): The employer did not verify that conditions in the permit space are acceptable for entry throughout the duration of an authorized entry: (a) On or about 03/01/2020 and at times earlier - Fuel House (a.k.a. Teepee), the employer did not verify that conditions in the permit required space such as working surface stability and energy isolation were acceptable for entry throughout the duration of the authorized entry, exposing employees entering the silo to engulfment, fall, amputation, and caught-in hazards.
