29 CFR 1926.1203(c): Each employer who identifies, or receives notice of, a permit space and has not authorized employees it directs to work in that space did not take effective measures to prevent those employees from entering that permit space: a) First Quality Tissue, Lock Haven, PA: The employer did not take effective measures to prevent employees from entering into permit spaces in that an employee was found within the weir section of the Dirty Tad Tank and was pronounced dead due to asphyxiation, on or about September 30, 2019.
