29 CFR 1926.454(b)(2): The employer did not have each employee who was involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the correct procedures for erecting, disassembling, moving, operating, repairing, inspecting, and maintaining the type of scaffold in question. Construction Site: On or about September 27, 2019, the employer did not ensure employees were trained by a competent person to recognize hazards and safely disassemble the tubular scaffold used onsite. This condition exposed employees to fall hazards above 10 feet.
