29 CFR 1926.1204(c)(4): Purging, inerting, flushing, or venting the confine space was not conducted by the employer as necessary to eliminate or control atmospheric hazards: a) On or about 9/20/2019, 304 Front Street, Binghamton, NY, Tank #2: An employee was required to enter a 10,000 gallon gasoline storage tank, to clean and prepare it for removal, The employer did not implement measures such as, but not limited to, forced air ventilation to eliminate the hazards of a flammable atmosphere, resulting in a deflagration and fire when an electric drill was used inside the permit required confined space. Abatement verification is required for this item.
