29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: (a) On or about September 20, 2019 at a worksite located at West Yellow Creek Field Tank Battery #3, Wayne County, Mississippi, an employee was exposed to a fire and explosion hazard while using an oxy-fuel gas torch to cut and remove a crude oil storage tank that had not been purged and cleaned for removal of residual flammable material. The employee was not trained to examine and test the interior of tanks to ensure they were free of flammable material.
