29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a. On or about August 7, 2019, employees were exposed to falling approximately 12 feet to the ground below when stepping off of the catwalk to load feed into trucks and fall protection systems were not used. b: On or about September 4, 2019, employees working in the Mixing Area were exposed to falling approximately 40 feet to the ground below because the manual hand-powered elevator did not have fall protection provided when operated.
