29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. On 16 September 2019, at approximately 8:00 AM, an employee suffered an injury requiring amputation of the left index finger. The amputation was not reported within 24 hours to the local area office of the Occupational Safety and Health Administration. The Austin Area Office was notified on September 17, 2019 at 12:56PM, by an employer safety representative.
