29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about September 12, 2019, the employer failed to ensure employee(s) were wearing personal protective equipment such as flame retardant shirts. Employee(s)were using portable torches to heat the soldered joint on a HVAC chiller line. There was an F-22 freon/oil mixture flash fire. Employee(s) were exposed to a burn hazard.
