29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On September 10th, 2019, at approximately 7:25 AM, an employee suffered an injury requiring hospitalization. The hospitalization was not reported within 24 hours to the local Area Offices of the Occupational Safety and Health Administration. The Austin Area Office was notified on September 11th, 2019, at 5:33 PM.
