29 CFR 1926.1209(j): The entry employer did not ensure that each attendant performed no duties that might interfere with the attendant's primary duty to assess and protect the authorized entrants: The employer is failing to protect employees from hazards associated with working inside a permit required confined space. This was most recently documented on or about September 6, 2019 when the hole watcher performed other tasks that interfered with the duty to protect the authorized entrants. The hole watcher climbed down from a work platform where the entry into the grain bin silo was located to assist another worker placing a ladder on the south side of grain bin silo #258.
