29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: The employer is failing to protect employees from caught between, crushing and struck-by hazards associated with the unexpected energization, start up, or release of stored energy. This was most recently documented at 10330 I St, Omaha, Nebraska on September 5, 2019 when an employee was repairing a John Deere Skid Steer without documented procedures to safely control the hazardous stored energy of the vehicle. The employee received fatal injuries when the unexpected loss of hydraulic pressure allowed the boom arms to fall on him.
