OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by and crushing injuries when not protected when standing in the fall shadow of granite slabs during handling. a) Warehouse - On or about September 4, 2019, Stone Kitchen and Bath Design Center, Incorporated dba Stone City failed to ensure an employee was protected when loading a 900 pound 65 inch x 125 inch slab of granite onto a A-Frame positioned on a flatbed truck. The employee was required to stand on the flatbed truck in the fall shadow of the granite slab and manually release the hoist clamp from the granite, exposing the employee to struck by and crushing hazards as the clamp became stuck and caused the slab to lean forward and fall.
