29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about 15 August 2019, a worker was hospitalized after being struck in the arm by a metal fragment and severing a main artery while at the worksite. The hospitalization was not reported to the Austin Office of the Occupational Safety and Health Administration within 24-hours.
