29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: a) Deer Lodge Medical Center: On August 18, 2019, and at times prior, the employer failed to report a fatality of one employee as a result of a work-related incident that occurred at the Powell County Fair Grounds, within eight (8) hours. An employee sustained fatal injuries on August 18, 2019 at approximately 9:30 PM and OSHA was notified by employer on August 19, 2019 at 1:45 PM.
