29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident. On or about March 5, 2019, an employee was initially hospitalized as a result of falling on the floor and striking his head near the condiment section of the restaurant. The sustained injuries later resulted in the employee's death on March 27, 2019. The employer did not report, within 8 hours, the death of the employee after receiving the amended death certificate from the Cook County Clerk's Office on July 22, 2019. There is no abatement certification or documentation required for this item.
