29 CFR 1926.501(b)(5): "Formwork and reinforcing steel." Each employee on the face of formwork or reinforcing steel was not protected from falling 6 feet (1.8 m) or more to lower levels by personal fall arrest systems, safety net systems, or positioning device systems: a) 6202 Germantown Ave., Philadelphia, PA 19144 - The employer did not ensure that employees who were walking on the face of formwork at heights up to approximately 9 feet above the lower level were protected from falling, thus exposing the employees to fall from height hazards. This condition was observed on August 16, 2019.
