29 CFR 1910.147(c)(7)(i)(B): Affected employees were not instructed in the purpose and use of the energy control procedure. Location: Worksite Employees were exposed to hazardous energy while working on a glycol system component where they has not been properly instructed in the manufactures energy control procedure. Actual Injury: On or about August 13 2019, employees were injured by flying debris when a rubber bladder exploded out of a pressurized vessel while preforming maintenance.
