Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.178(o)(1): Load(s) on powered industrial truck(s) were not stable or safely arranged: (a) Facility, 107 Highland Street, Martinsburg, PA: On or about August 9, 2019 a sole press being moved by a powered industrial truck as not stable or safely arranged. The press had a severe hydraulic leak and oil soaked pads were under and around the machine. The press had two lags bolts, one in the front and one in the back that was holding the machine in place. The bolts were covered by the absorbing pads and were not visible. As the operator went to lift and move the load it shifted and tipped over onto an employee who had been next to the machine. The employee sustained broken ribs and a laceration to their left ear.
