29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a) APM Terminals, 901 Ezra Trice Blvd, Mobile, AL: On August 4, 2019, the employer had an employee that got struck in the face by a twist lock that had fallen approximately 25 - 30 feet, resulting in injuries that required hospitalization. The employer failed to report the work-related hospitalization to OSHA within twenty-four (24) hours.
