29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Production Area: The point of operation on the Amada 80 ton Mechanical Press Brake Model #8024LD (Serial No. 80240077) was not guarded while employees were operating the press, exposing employees to an amputation injury during the operating cycle, on or about May 10, 2019. In accordance with 29 CFR 1903.19, abatement documentation and certification is required for this violation. Pennmark Technologies Corporation was previously cited for a violation of this Occupational Safety and Health Administration standard or its equivalent standard, 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1142575, citation number 2, item number 1 and was affirmed as a final order on July 12, 2016, with respect to a workplace located at Lower Demunds Road, Dallas, PA. Pennmark Technologies Corporation was previously cited for a violation of this Occupational Safety and Health Administration standard or its equivalent standard, 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1048919, citation number 1, item number 2 and was affirmed as a final order on September 15, 2015, with respect to a workplace located at Lower Demunds Road, Dallas, PA.
