Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1904.39(a)(1): Basic Requirement. Within eight (8) hours after the death of any employee as a result of a work-related incident, you must report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor. The employer failed to report a work-related fatality within 8 hours of learning of an employee who was operating a Yamaha Rhino UTV Model# R66 on July 18, 2019, at job-site address 5640 Lower Valley Pike, Springfield, OH 45502. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET)
