OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushed-by hazards while attempting to free the platform of a articulating boom aerial lift that had become jammed or snagged in the overhead structures of a building. a) Jobsite, 1190 Midland Avenue, Midland, PA 15059: On or about August 1, 2019, two employees were exposed to struck-by hazards while attempting to free a JLG articulating boom lift (model number 800S, serial number 0300172497) caught on a structure near the ceiling while employees were still in the lift.
