29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: On 3/25/2019, an employee of John Beal Inc. was repairing roof shingles on a residential building located at 2101 Preston Woods Parkway, Lake Saint Louis, MO. The employee working on the roof was using a ladder to access the roof which was not extended three feet beyond the landing or edge of the roof.
