29 CFR 1910.67(c)(2)(ii): Only trained persons shall operate an aerial lift: On or about July 19, 2019, the employer failed to ensure that formal instruction and/or training was provided to an employee who was using a rented articulating boom lift (JLG 400S) during service and maintenance activities. The untrained employee tasked with servicing an industrial baler was working from the basket of the JLG at a height of approximately 13 (+) feet.
