29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a. On or about July 6, 2019, the employer did not ensure that the Rapid Pack Line 5 machine was adequately guarded in that the opening near the dater head was large enough to allow employees to reach the sealing die mechanism. b. On or about July 6, 2019, the employer did not ensure that the Rapid Pack Line 3 machine was adequately guarded in that the opening near the dater head was large enough to allow employees to reach the sealing die mechanism. Fresh Mark, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1283608, citation number 2, item number 1, and was affirmed as a final order on May 28, 2019, with respect to a workplace located at 1600 Harmont Ave, Canton, OH 44705.
