29 CFR 1904.29(a): The employer did not use an OSHA 300, 301 and 300A Form or equivalent. a) Monarch Towel Company/311 Hollywood Ave. South Plainfield, NJ 07080- Throughout the facility: The employer failed to maintain OSHA 300, 301 and 300A the injury and illness records at the establishment. On March 15, 2019 employer had in-patient hospitalization involving a work-related injury. Condition noted on or about July 10, 2018.
