29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing and labeling. a) On or about, 05/21/2019, in the Loading dock (Door #2), employees were exposed to burns due to electrical fire when using the portable conveyor belt after the connector plug was replaced with the wrong plug. The conveyor belt was rated for a 20A, 3 Phase 250 V AC connector plug but it was incorrectly replaced by a 20A, 125/250V AC plug connector. NO ABATEMENT CERTIFICATION REQUIRED
