29 CFR 1910.23(b)(8): The employer did not ensure ladders were used only for the purposes for which they were designed: The employer is failing to ensure that employees are protected from fall and struck-by hazards when using a ladder that has been modified. The most recent occurrence was evident during the inspection conducted on July 10, 2019 at 1200 Centennial Rd., Wayne, NE 68787. The employer has employees using ladders in the Quality Control department to gain access into the back of semi-truck trailers that have been modified in-house. The employer has added a rolling platform on the bottom and extendable wings on the top. These modifications have not been approved by the manufacturer, nor have they been certified by an engineer.
