29 CFR 1910.23(d)(7): Grab bars did not extend 42 inches (1.1 m) above the access level or landing platforms served by the ladder: a) Worksite: Employees operating the McCarter Iron Works 20,000 lb. horizontal tank/chocolate melter were required to access the platform using a fixed ladder that did not have a grabrail on one side; on or about 7/9/19. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
