29 CFR 1910.146(d)(1): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not implement the measures necessary to prevent unauthorized entry: a) Jobsite Located at West Rively Avenue and South Sycamore Avenue, Alden, PA: The employer did not implement adequate measures to prevent unauthorized entry into permit-required confined spaces. Two employees were asphyxiated when they entered an oxygen deficient sewer manhole where as the O2 levels were at 8% at the bottom of the manhole and 13% at the entry point. Documented to occur on or about 11 JUL 2019.
