29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l): 1) Warehouse Area, Near Aisle 34 and 36: On or about July 2, 2019, the employer exposed an employee to struck-by hazards by not training employees operating the Raymond Stand-up #25 model 425-C30QM and serial 425-13-39052 while loading and unloading materials in the warehouse areas. The Bunzl Distribution was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(6), which was contained in OSHA inspection number 1071489, citation number 01, item number 003 and was affirmed as a final order July 22, 2015, with respect to a workplace located at 1915 E. Florida, Springfield, MO 65803.
