29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a. On or about 9:00 AM on April 10, 2019, at 7916 US Hwy 27 S, Sebring, FL 33876, an employee suffered a work related injury and was hospitalized. The employer was aware of the in-patient hospitalization in the afternoon of April 10, 2019. The employer notified OSHA on April 15, 2019 at 11:55 AM of the in-patient hospitalization. Circle K Stores, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1904.39(a)(2), which was contained in OSHA inspection number 1241092, citation number 1, item number 1 and was affirmed as a final order on 07/24/2017, with respect to a workplace located at 2101 SR 19, Tavares, FL 32778.
