29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: a) At the facility, where the employer did not report the hospitalization of an employee within the twenty-four hour requirement. An employee was hospitalized on May 27, 2019, after having his foot crushed between an electric pallet jack and a racking system. The hospitalization was not reported to OSHA until June 5, 2019.
