29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval and capacity ,operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly: Block Machine: On, or about, April 4, 2019, a boom attachment was used with the Yale fork truck, serial number D875V04302N during a gear box overhaul project. The fork truck was not marked to indicate the reduced capacity of the fork truck when the boom attachment was used.
