29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Such activities were not performed only by experienced and trained employees selected for such work by the competent person. Underside of Bridge, State Line Rd and I-90 at Mile Marker 1.8, West Stockbridge, MA On or about June 12, 2019 during three days of dismantling of Safway Quikdeck suspended scaffolds on both sides of the bridge, the employer had not assigned any competent persons to supervise and direct the operations. The foreman and employees had never reviewed the manufacturer's Assembly or Safety Guides, were not knowledgeable in identifying or predicting hazards, and did not recognize that they were using improper procedures and equipment. On June 5, 2019 the second day of the dismantling, two employees were dismantling the scaffold with no competent person on-site and the employees were injured when the scaffold shifted.
