Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that an employee left hand was exposed to a caught-between hazard from exceeding a 1/8-inch clearance between the table, disc surface and abrasive pad of a Max Universal 24-inch disc grinder. According to American National Standard B11.9-2010, section 8.3.2.2 a 1/8-inch clearance was required between the abrasive product and the table. a) NNS, Building #5: On or about 06/03/2019, a Max Universal 24-inch disc grinder used in grinding metal plates, the distance between the table, disc surface and abrasive pad was not at a maximum 1/8-inch, as a result an employee's left middle finger was partially amputated. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
